NOTE: Covered retirees, spouses and adult dependents should only attend the PERA, PAB or CUB locations, or use the Personal Physician Form and visit their own doctor.
If you are unable to attend a WellSMART event, you do have the option to visit your physician to have your fasting lab work completed. You will find the physician form by visiting the main page of the Wellness Center and clicking on tile labeled WellSMART. Follow the instructions found on the form to ensure you receive credit for completion of this activity.
